Nursing Process

What is nursing process?

Nursing process is a patient centered, goal oriented method of caring that provides a frame work to the nursing care. It involves five major steps of assessement, nursing diagnosis, planning, implementation/intervention and evaluating.

The nursing process is based on a nursing theory developed by Ida Jean Orlando. She developed this theory in the late 1950's as she observed nurses in action. She saw "good" nursing and "bad" nursing.

The following are the steps or phases of the nursing process.


  • Assessment (of patient's needs) - This is the data collection step.
  • Diagnosis (of human response needs that nurses can deal with) - NANDA
  • Planning (of patient's care) - goal setting SMART
  • Implementation (of care)
  • Evaluation (of the success of the implemented care)
